Through Wellthy’s Care Concierge, you’ll be personally matched to a Care Coordinator to best support your care needs. Your Care Coordinator will work with you to identify priorities, create a personalized plan, and manage the tasks on your plate. You can use the Wellthy platform to keep everything organized, track progress, and keep your loved ones in the loop.
Wellthy’s Care Team is made up of experienced professionals from healthcare, social work, mental and behavioral health, aging, and patient advocacy. Every Care Coordinator brings at least five years of experience, with many holding advanced degrees, so you can feel confident you’re getting knowledgeable, thoughtful support.
